What kind of corrective eye surgery can cause monovision?

I've heard that some kinds of corrective eye surgery can cause monovision. But do you know what kind of these surgeries are?

    I've heard that some corrective eye surgery can cause monovision, such as LAIK, LASEK and PPK. But this rarely occurs, so there is no need to worry about it.

    Some people say cataract surgery with intraocular lens implantation can cause monovision. Before you have eye surgery, you should talk to your doctor with the possibility that it may occur in the procedures.
